The Expanded Program on Immunization (EPI) has been one of Pakistan’s major healthcare initiatives, and has done a great deal of good. The devolution of power provided the provinces with autonomous planning. However, it also revealed institutional and capacity challenges in the health services management and delivery. Based on the results of the recent analytical work, and a lively process of consultations with World Bank partners in Pakistan, this article identifies a few factors that influenced the underachievement of EPI health indicators, including weak financial management and accountability mechanisms, as well as discusses the possible ways forward.
